You're going to need to just go ahead and have the surgery, and figure out how to pay for it later.
If you truly can't afford health insurance (only $100/month or less for someone 22 years old) then you really should be eligible for Medicaid. Of course, you needed to have been responsible and made sure you had coverage before you actually developed a medical issue. At this point you should just go get the surgery and then work out a payment plan with the hospital. Once you pay off the surgery then work a second job so you can buy some health insurance so you have it before you actually need it.
If you live in U.S.A you are out of luck. Try to see if you can make a trip down to like France or Great Britain, Cuba, or even Canada.
Personally, you should see the movie 'Sicko' by Michael Moore.

If you go to a County hospital, they will charge you what you can afford and let you make payments. The doctor may not be as agreeable.
