If you lost coverage due to not having regular care and it was within a typical six months you may not have to reapply. You may request your case be reopened with the medical records and form they previously requested. A really long letter would have been sent after your claim closed explaining the dispute process and how to reopen the file. Call the disability specialist or company you had before to start the process and get the forms. We had to verify disability every three months.
Make sure you do this right away or you may not be covered!

Most disability plans require that you be under the regular care and attendance of a physician.
Sorry to say, but you will have to go through all that stuff again. if you qualify when you reapply.
It will depend on if you still have coverage, on if you are disabled enough to qualify, and why you were cut off of it before.
